  • I hereby acknowledge that participation in this basketball camp involves an inherent risk of physical injury. In consideration of this registration being accepted I hereby for myself, my heirs, executors, administrators and assignees, waive any and all rights and claims for damages and losses I may have against Novi Community School District, all Area 4 Star basketball Camp, Coach Schluter's Basketball Camp and camp staff(coaches, trainers, volunteers) for any and all losses or injuries suffered by myself and/or my child during the life of this program. My child is in good health and is covered by my insurance for any injuries that he or she might sustain while participating in the camp. I authorize the camp staff to act for me in their best judgment in the event of any emergency requiring immediate attention. All refunds requested prior to the start of training will be granted $100. All refunds requested the first day of training or later will not be refunded. If you sign up for an individual session(s) and cancel, no refunds will be given. I approve for pictures of my son/daughter to be posted on the Coach Schluter Basketball Camp website.
